• used for electing new members on a yearly bases before the Annual General Meeting

Membership Form: Membership

• To become or remain a member of the association, dues are yearly

Forgery Alert Form:Prescription Forgery Alert

• Completed when the pharmacist or technician confirms a prescription is a forgery

Lost Prescription Form:Lost Prescription

• Completed by either the pharmacy when the client informs the staff of the loss

Prescription Forgery Alert Program

One of HDPhA’s most significant projects includes the implementation of a Forgery Information System. Over 250 pharmacies participate in the program to reduce the number of forgeries in their community. The organization uses a fax fan out system to pharmacies to disseminate information of suspicious prescriptions. As a result of this program, they have been able to obstruct almost 80 forgeries over the past year. In addition, HDPhA has developed a working relationship with the Hamilton Police Department to create awareness of their efforts to reduce forgeries and their work to keep narcotic/controlled substances off the streets and out of schools. HDPhA continues to meet with representatives from the Police Department to discuss better cooperation between pharmacy and the police.
